How do I register?

These must be completed and handed in to a receptionist in order to join the practice.

All patients aged 5 and over will be required to make an appointment with the Nurse for a New Patient Check.

It is vital that you attend for a check as information can be obtained which will ensure that you are offered the most appropriate services.

Practice Area

The Practice serves Ainsdale with a Northern limit of Carr Lane/Windy Harbour Road.

To the South the boundary is Moor Lane, Woodvale.

Included to the East are Shirdley Hill and Halsall

Scarisbrick and Haskayne are not included.

Please inform reception if you move outside the practice area.

Patients already registered who move outside Ainsdale may remain provided their new address is South of Grosvenor Road, Birkdale.
