Download our New Patient Forms HERE. The download includes both the NHS standard registration form and the Ainsdale Medical Centre New Patient Questionnaire. Both these forms must be completed and handed in to a receptionist in order to join the practice and book an appointment.
Only people who are ordinarily resident within our practice area are eligible to register with us. Click HERE to see a map of our Practice area.
The boundary rules are laid down by NHS England and we are unable to make any exceptions for people living outside the boundary.
All new patients aged 5 and over will be required to make an appointment with the nurse for a new patient check.
It is vital that you attend for a check as information can be obtained which will ensure that you are offered the most appropriate services.

A Patient Guide to GP Services
If you are ill while away from home and staying at an address within our practice area you can receive treatment at this practice as a temporary resident.
You can be registered on this basis for up to three months. This will allow you to be on our list and still remain a patient of your permanent GP. If you expect to be resident in our area for more than three months you should register with us as a full patient. Please note this will mean that your registration with your current GP surgery will end and your full medical history will be transferred to us.
To register as a temporary patient simply contact us to tell us why you need to be seen by a doctor.
